The document provided isn't the final report, so I can't say if there's anything inaccurate about it.  Maybe they uncovered a major issue, but this preliminary report is too vague to verify anything.  They claim all kinds of issues, but provide no specific examples.

Not a good start for the report.  They already changed their cover page after their initial claim/picture of a vote coming from a supposedly empty lot was debunked in a matter of minutes after the report was released.  Old cover page had 2058 E. Wildermuth Ave in Tempe, AZ. New one has an address in Goodyear, AZ claiming (again) a vote came from an empty lot....which it didn't. A simple search on Realtor.com shows a house on the 4.3 acre lot.
