This is UNIVERSAL with Law Enforcement, the name of the report might differ but yes, every time you interview a witness, suspect, gather a piece of evidence IT MUST be documented, most investigators also make written note's
so the hand written notes are often photocopied. All of these REPORTS go into a a CASE FILE JACKET....subject to REVIEW, periodically by a SUPERVISOR. INVESTIGATIVE CASE JACKETS are usually kept under lock and key with limited access....DETERMINED BY THE CASE AGENT.
Now back in my day, reports were done on a form that had five carbon copies. Said copies were supposed to be disbursed throughout the Dept to include records and Statistics, AND AVAILABLE TO THE PUBLIC, however, ongoing investigations or "sensitive" cases were not disbursed of course until after a Trial or what have you.
I imagine nowadays that everything is done on computer and in a zip file or data storage device and only printed out going to court. I don't know what the procedure for the FBI is, but I would imagine there is some "Hard Copy" of all material in a case file jacket. It might be easy to change a computer generated report but a "hard copy" would have to be pulled and destroyed or amended BY DATE.
Last but not least all of the aforementioned material is DISCOVERABLE EVIDENCE by the suspect/accused's Attorney.
If there are any changes, redactions or 'anomolies' the Attorney is going to jump on it in court.
You get some idea of how Investigative files are compiled and maintained. Pay attention to that part about SUPERVISORY REVIEW and approval of all reports....there is a paper trail....WITH SOMEONES NAME ON IT.
